Radiology Management System

The HIMS™ Radiology Management System (RMS) are available as standalone systems or as integrated components of the HIMS™ product suite. The RMS receives patient demographic, order and appointment information from the Registration and ordering components of the Clinic, Ward and Radiology systems. The Radiologist workstation has been designed to optimize Radiologist workflow. All recurring transactions are automatically generated each payroll period. Non-recurring transactions such as overtime are captured in the Rostering module and can be posted directly to payroll upon approval.

Financial Module

The HIMS™ Financial Module is the accounting interface between the HIMS™ front and back office systems. It represents the main integration point for business processes, concentrating financial transactions generated during a patient visit into financial data that can be posted to the General Ledger via the Accounts Receivable gateway.

The HIMS™ Financial Module gathers all real time inpatient and outpatient charges and doctors’ fees generated across the hospital and allows them to be prorated and invoiced to all payors associated with that patient’s visit. Fully integrated with all other HIMS™ modules, the Financial Module provides hospital users and management with instant access to the patient’s entire charge and payment status. The Financial Module operates in real time, updating all patients billing information across the system 24 hours a day, 7 days a week.

The Financial Module provides a suite of claims management tools, which allow the utilization of patient demographic and billing information, coupled with the charges generated across the system to submit and follow up on all claims submitted to an unlimited number of payors as assigned to the patient’s visit. A powerful centralized policy set up tool allows departments to maintain coverage, limits and exclusion billing rules for each payer, contract or benefit scheme and apply these to any patient subscription.

The Financial Module is also an integral part of the materials management process within HIMS™. Material or value flows are recorded throughout the system and the correct allocation of all payments is required to credit and debit the cost centers concerned. This directly links the HIMS™ patient billing process to the Inventory and Purchasing Modules and enables the automation of both processes without placing additional data entry responsibilities on hospital personnel.

Fixed Assets

The HIMS™ Fixed Assets module integrates seamlessly into the HIMS™ Back Office suite. Taking the master asset information from the Purchasing module via Accounts Payable invoices, the Fixed Asset module completes depreciation runs and then feeds the information automatically into the General Ledger.

The Fixed Asset module supports automatic asset take-on from the Accounts Payable module, multiple asset ledgers and a master asset ledger for asset data retention and depreciation forecasting.

The Asset Core workbench contains powerful tools designed to allow users to find assets efficiently, with all details relevant to the asset easily accessible from a central location.

Transfers of fixed assets between locations, cost centres or departments; reclassification of assets and reassessments of asset values are all easily performed.

The HIMS™ Fixed Assets module is designed to be database-friendly, despite the large amounts of data it must process. All depreciations for the entire lifetime of each depreciable asset is stored in the database. This, together with other optimizations, makes the depreciation of 30,000 assets run in under 10 minutes, and still enables other transactions within HIMS™ to function "as usual".

In parallel with the Main Type there is also the Asset Category, which holds default values for GL accounts, lifecycle and depreciation method.
Smart Search Intuitive asset search by tag number, serial number, description, internal reference number etc.. A search screen displays all possible matches if more than one result is returned.

Materials Management

Inventory

The HIMS™ Inventory module, coupled with the Purchasing module, provides a complete Materials Management Solution for hospitals. Completely integrated with the HIMS™ system, Inventory automatically records and manages in real time all changes in stock levels throughout the hospital. Whenever an item is sold, dispensed within the hospital or has its stock level adjusted, its movement is immediately recorded and simultaneously visible to Inventory users. As items are charged to a patient's account, they are automatically deducted from the inventory of the providing store.

Replenishment between stores occurs either automatically or semi-automatically, as set up per item per store. When an item reaches a defined minimum level, a purchase request is automatically available for processing in the Purchasing module. All inventory movements are posted to the General Ledger.

System Network & Architecture

HIMS™ was designed according to the following stringent goals:

  • Highly Available and Redundant - At the client tier level, there are other machines that are necessary to perform the daily operation of the hospital. If one machine fails, the others can take over the workload of the failed unit.

    Database Server Architecture

    HIMS™ is designed specifically to keep all records online indefinitely. This strategy allows reports to be created using historical data. Any database housekeeping tasks are performed online, without any downtime and with negligible system impact. No records are ever purged, deleted or moved offline for any reason. Patient data remains available online for the lifespan of the system.

    Availability Two main database server configurations provide different levels of reliability and overall system uptime. The Single or Clustered Database Server configurations both use database servers to access the single Fibre Channel (F.C.) Storage unit.

    Clustered Database Configuration Using a clustered database server configuration ensures HIMS™ commitment to high availability. The cluster comprises two servers connected simultaneously to the Fibre Channel Disk system. If one of the servers should fail, the other server takes control of the disk system, returns it to a known safe state and continues processing as normal. A slight delay is seen as the server change takes place.

    Single Server Configuration The single database server configuration works on the same principle, but does not have the redundancy advantages provided by the cluster configuration. In the event of server failure, the system will cease to function until the failure can be fixed.

    Comlogik Business Systems, Inc. strongly recommends RAID 10 (also known as RAID 1+0 or RAID 0+1) as that reduces the load on a system with heavy updates such as HIMS™. Disk speed is an important factor when designing a database server. Currently, 10,000-RPM disks are the standard, though 15,000-rpm disks are now available, though only in smaller capacities.

    Disaster Recovery

    DLT tape backup units provide cost effective storage for large volumes of data, but they are not fast enough to provide an acceptable backup and recovery strategy. The time taken to restore a large database is already widely recognized as the ‘weak link’ in the disaster recovery plans of many organizations. Consequently, Comlogik Business Systems recommends using software to perform real time data replication to another remote online backup server.

    Real-time data replication to geographically distinct online RAID storage arrays means that in the event of a total failure, the system can be brought back on line within minutes as opposed to hours of a failure. Current Fibre Channel technologies allow additional clusters to be located a number of kilometers away from the main site without any degradation in performance. Similarly, in areas where the appropriate infrastructure is both available and dependable, wide area networks or other high bandwidth network solutions can be used thereby increasing the geographical separation between the production and backup centers and increasing the data security.

    For additional security, the secondary or backup server can itself be written in a cd or separate hard drive which can then be stored offsite providing yet another level of data protection.

    Similarly for small or medium hospitals, backup server can be located near the main site as long as this will not be affected when fire or other calamities strikes.

    Hardware

    The selection of appropriate hardware is critical for any installation of a system the size of HIMS™. At Comlogik Business Systems, Inc., experience has shown that a number of common-sense practices can ensure that the hardware you choose is right for your hospital and location. When selecting hardware, consider more than just the initial purchase price of the equipment. All hardware selected should be:

  • Manageable - If all of the devices report to one central administrator console, problems can be discovered as they occur. Proactive maintenance is far better than waiting for a service call.
  • High Performance - Seemingly similar products can often vary significantly in their ability to perform quickly and efficiently. Careful hardware selection means a much longer product lifespan and therefore considerable cost savings.
  • Accompanied by Strong Technical Support - Knowledgeable technical support can make the difference between a successful rollout and mediocre one. Web based and telephone support is essential.
  • Up-to-date - For example, if a server manufacturer can release his GHz processor machine to market 3 weeks before the competitors, he stands in a very strong position.
  • Readily Available - Do not buy hardware that is hard to obtain in your current location. Ask the vendor if they have in-country stock. Waiting for an international shipment is not an option.
  • Consistency When planning the installation of a piece of software the size of HIMS™, it is best to standardize by using the fewest different types of equipment possible. Consistency is king - instead of buying 3 different sizes of label printer to suit different labeling needs, one brand and one model mean that the customer need only sign one support contract and troubleshoot, maintain, update and repair one kind of machine. This also enables departmental staff to become 'experts' in one machine rather than knowing a little about many.

    Purchasing Considerations Buy commonly available, commodity products only. Specialized equipment is generally unnecessary. In addition, keep a supply of the commodity products on site - even if you sign a support contract, vendors usually promise 4-hour response time. With spares on site, hardware can easily be replaced with a minimum of downtime and interrupted operations.

    Client PCs When buying Client PCs for your user's desktops, ensure that the vendor will replace any machines that fail within one business day. CPU speed is often a major selling point for desktop machines, but there is really no need to buy the fastest possible chip if it is going to be used as a HIMS™ terminal. The Intel Celeron is excellent in terms of price/performance. As for system requirements, ensure that each machine has at least 2 USB ports and a minimum of 256 MB of memory. RAMBUS and DDR memory are the new options for the faster desktop PCs.

    File Server When buying a File Server, it is common for people to fall into the trap of selecting a server class machine with a powerful CPU. For example, a dual Pentium III is often sold as a minimum specification. This is not necessarily, what you need - retrieving files is not CPU intensive. File servers generally follow the 80-20 rule. 80 % read operations, 20 % write operations. For this reason, a single processor machine is more than adequate for the needs of most hospitals, especially with the increasing capabilities CPUs available on the market today. Another important thing to remember when purchasing a file server in today's market is the advancements in the operating systems that run them - Windows 2000 actually reduces the CPU's load even further, promising greater performance on all machines.

    Database Server When buying a Database server, there are a number of features that can significantly increase the availability.

  • 24 Hour support from the hardware vendor with a 4-hour call out 365 days a year.
  • Adaptive Load Balancing / Fault Tolerant Network Interface Cards. Intel currently makes the best Fast Ethernet and Gigabit NICs in the world.
  • RAID 1+0 disks

  • Disk speed is an important factor when designing a database server. Currently, 10,000-RPM disks are the maximum available and highly recommended. In the near future, 15K rpm disks will be the new high-end speed.

    Fibre Channel is a technology used to connect hard disks to servers. All of the hard disks are dual-ported and are connected using an Arbitrated Loop (FC-AL) this provides redundancy and higher throughput.

    HIMS™ security is addressed in two distinct ways: through the use of native Windows 2008 security features and through HIMS™ application security.

    Windows 2008 Security

    HIMS™ uses the Windows 2008 advanced security features. These features include the following:

  • Login Security - All HIMS™ applications natively use Windows 2003/2008 authentication. This is based upon Kerberos 5, an open standard security system. Kerberos 5 is highly secure and was designed by Berkeley. It handles the logon of the users and the permissions they have to files over the network. For those users that share a workstation, HIMS™ applications can log on as individual users, all sharing a single desktop. By implementing tight integration with Windows 2003/2008 security mechanisms, HIMS™ automatically inherits Windows 2008's security features and benefits.

  • Network Security - HIMS™ inherits the rich network security features built into the Windows 2008 environment. Features such as IPSec and various encryption and VPN options are available.

  • Data Encryption - All data passed to the clients is encrypted. The encryption mechanism is configurable but currently RC4 (128 bit) is used with 3DES (168 bit) as an option.

  • Caching - Data is rarely cached on clients. Where there is caching it is only for temporary data retrieval. Such data is always deleted.

  • Active Directory Integration - No employee passwords are stored or displayed anywhere in HIMS™. The login mechanism is based entirely on Active Directory. Client applications are protected at the process level. Multiple users can sign on to a single workstation each with a different process running as the relevant user. If users wish to temporarily leave their applications open they can lock their processes.

  • Database Access - The database server has only one user account - the system administrator. Users cannot connect to the database using their Active Directory login.

    HIMS™ Application Security HIMS™ allows full or partial access to all patient data, depending on the login rights of the user at the application software. The concept is based on “roles” and allows granular patient-physician associations to ensure the appropriate access to the interested parties. The HIMS™ system security and user access is managed using the HIMS™ Security Administration function. It has the flexibility to control access through a number of groupings and the ability to define Read-Write or Read-Only authorization on a user group or individual user basis.

    User Groups are central to HIMS™ security. User Groups allow the control of access to many areas of HIMS™ and dictate the type of access rights and privileges a user is granted. User Groups allow access to be given to specific functions within a module, access to information from specific departments define the rights and privileges of specific job types and control the types of information a user is able to read, create or print or edit.

    HIMS™ Audit Security In addition to the Security provided by the security module in HIMS™, the system logs all access to all parts of the patient data.
